Listed below are the documents from this link, schools.nyc.gov/EnrollmentForms, that you need to fill out for registration.  Once you open them online (you may have to save them to your computer first and then fill them out and save the information), complete the forms, and bring them with you for in-person registration.

1. Student Registration form 

2. Home Language Survey - HLIS 

3. Emergency Contact card 

4. Housing Questionnaire 

5. Parent/Student/Guardian (PSE) Ethnic & Race Identification form 

6. New Admit Kindergarten Questionnaire                                                                                       Also, we need the proofs that are necessary for registration.  Please scan them or take a clear picture of them (one at a time) so that I will be able to see the information.  I would need the following proofs:                                                                                            

7. Three (3) proofs of address in your name (acceptable proofs are listed on your offer/waitlist letter                                     

8. Child's birth certificate 

9. Child's proof of immunizations (I need to see all immunizations given and the date that they were given) 

10. Copy of child's IEP (if applicable) 
    
    If you do not have a scanner/printer, please take a clear picture of these documents with your cell phone and then email those along with your filled out documents.
